How to Prepare

While we accept walk-ins and you are welcome to just come in, there are a few things you can prepare ahead of time before your first visit to make the process easier.

Call & Make an Appointment

Call 712-580-4570 to make an appointment. If you call after hours, leave a message and someone will return your call the next business day. We also accept walk-ins as the schedule allows.

Complete Forms

If you'd like to save time at the office, you can click the button below to print and complete your forms prior to arriving for your appointment.  You may also complete the forms in our reception area by arriving 15 minutes early for your appointment.

Have a Purpose in Mind

Whether it's a general health checkup or a visit for a specific condition or problem you are experiencing, be prepared to communicate your needs & concerns at the visit.

What to Expect on Appointment Day

What can you expect when you walk through our front door? A friendly staff, a warm atmosphere, minimal waiting times, and quality time with a provider.  There will be forms to be completed and insurance information to provide, and any copays that your insurance requires will be collected at the time of service.  Following your appointment, you will check out with our front office staff, who can answer any billing or insurance questions you may have.  If further appointments are necessary, either with Priority Health Northwest or a specialist, that will be handled at that time as well.


If you are not able to make your appointment, please call and cancel as soon as possible in order to make that appointment slot available for someone else.

Follow Up and Everything After Your First Appointment

At the conclusion of your appointment, we will answer any questions that you have and make sure that you understand the discharge instructions and plan going forward. We will inform you of lab or imaging results as soon as they are completed. The results of most labs or x-rays taken in our office will be available prior to the conclusion of your appointment and will be discussed with you before you leave. Otherwise, we will call you with the results and discuss any further follow-up at that time.
