Insurance Plans

Health plans which we accept:

It’s important to know what your insurance company covers and any requirements it has for services to be covered. Your insurance plan should outline for you what is covered by your plan and what isn't.  Some plans require you to see a provider/clinic that is in their network. For some insurances, if you go to an “out-of-network" provider/clinic, it could mean that you will have a higher cost share for those expenses.

When you check in for your appointment, you will be asked to provide your insurance card and/or verify your insurance information on file is still current.  Some insurances have copays for particular services, and copays are a part of your contract with your insurance company and are payable at the time of service.  

Priority Health Northwest will bill your insurance company on your behalf.  You will then receive an explanation of benefits from your insurance company, which details what amounts have been applied to your deductible, paid to the provider, and the balance that you owe.  If your explanation of benefits shows that you owe a balance, then you will receive an easy-to-read billing statement from Priority Health Northwest with instructions on how you can pay your balance and when that balance is due.
